Above is a Google Form I made to do audits within the staff. We do audits to make sure we aren’t overusing certain sources or neglecting others. You can’t see in the photo above, but below the visible questions there is also one relating to gender and another to affiliation (a sport, a club, etc). I make sure audits are on the agenda almost immediately after every issue, and the class does them all at once. I compile the results and use them to make a Blacklist of students, teachers, and other members of the community that are banned from use by the staff unless absolutely necessary.
